PHP Fast & Easy uses MySQL and teaches you more than enough to use MySQL and PHP together, and EASILY at that.

Web Database Applications with PHP & MySQL by O'Reilly.

If you need to learn some of all three (PHP + MySQL + Web apps) this book would be a good fit.

I think I could recommend O'Reilly (Programming PHP, PHP Cookbook, Managing and using MySQL...) I have almost always been satisfied with their books (apart from UML in a nutshell which was impossible to understand)
