PHP CRM applications

Recently, I came new to CRM.
I've lot of questions.
Being a PHP programmer, I intend PHP CRM solutions.

Few routine questions:
How can we develop VOIP in custom code CRM?
What is better if we use existing product like SugarCRM, Worlddo or NET CRM or other?
What has Asterisk has to do with SugarCRM, telephony/VOIP
and IBM?

I saw some code on the SugarCRM site to allow screen pops from asterisk. If you're just getting into it, Asterisk @Home is a live CD that installs Asterisk + Sugar, among other things.
