mainpage for every directory index on normal page?

Hi,
My site needs an reestructuration so I want to put diferents subpages from the homepages.
Not to have all pages linked from the homepages.
(Have 6 indexpages already, diferent languages and diferents for rentals and sales.)

I see many sites always call the mainpages in each directory index.htm, but many sites dont.

What would be the best? I see it more logical each directory own index page.
Could it be seen as spam having to many index pages in an domain?
For exampel travel in Los Angeles,
so you will have
homepage - los Angeles - domain.com
homepage - los angeles east - domain.com/los_angeles_east/
homepage - los angeles west - domain.com/los_angeles_west/
and so on

or would be better: domain.com/los_angeles_east/los_angeles_east.htm?

domain.com/los_angeles_east/los_angeles_east.htm

This includes more keywords which can be an advantage.

I think this looks spammy: domain.com/los_angeles_east/los_angeles_east.htm

By the other hand, at least for my site, google gives index pages higher ranking than normal pages.
So would be an advantage an index page,

But the most importance isnīt the advantages is more not to be spammy in searchengines eyes, think I read somewhere they donīt like to many subdomains in an domain, should be the same with directories and index pages I suppose?
How many indexpages on an domain is to many?
