When site's cache refreshes, is this when Google changes your rank?

Quick, question. I am confused. When the cache under your website refreshes, is this when Google will re-index your site giving you a different ranking? The reason I am asking is because I added my main targeted keyword to my index page evenly spaced throughout the page... I then waited several days. Finally my cache refreshed. I took a look and I am still in the exact same position for all of the keywords. Any clue why I didn't change rank? Thanks.

As I saw for my pages:

1-spidered
2-indexed (changing cached version)
3-rank position settled, I think when pr was calculated

This process takes a month and restarts every 10 days -average-
So pages entering in the index could be re-crawled at same period.
