1) Someone types the URL or copies and pastes it from somewhere.
2) Someone comes in from a bookmark. (Systems used to track these pretty well, but many don't anymore due to changes in the way bookmarks are stored in newer browsers).
3) Someone is using Norton or some other software that blocks referrers.
4) A Spider/Web Crawler hits. (They never have a referrer)
5) E-Mail/Newsgroups (Web based e-mail will give a referrer but depending upon your tracking software, browsers [Outlook, NS Mail, Eudora, etc.] may not send a proper referrer).
6) There are more, I'm sure, what am I forgetting?
