Im looking for information about how to make a user survey. What you should think about, for how long you should have it, etc.

Anyone know a book or an Internet resource on this subject?

What is the purpose of the survey? Site feedback? Demographics?

I would keep it short (5 questions max). I recommend against prizes because it generates lots of false information. Run it until you feel you have a good sample to base the responses off of.
