Hello,

I have several hundred affiliates. Each affiliate is given a particular ID. The URL looks like this: xyz.com/index.php?=affiliateID. I noticed today that Google has my site listed only with the actual URL (no title no description in the serps) and Google partners such as AOL have completely removed my site from the search results.

Does anyone have any proven advice as to how to fix this issue?

I think the first mistake is assuming you know why you got banned from Google. What exactly is your evidence that it had anything to do with your having affiliates? And even if you had evidence, evidence is not proof.

Making assumptions is very dangerous. I'd start over and begin at finding out WHY you might have been banned rather than how to address the *assumed* reason for your banning.

There's a good thread here about getting back in after banning, search for something like "a banned site checklist".
