Does the ammount of times my favicon file has been accessed equal the ammount of visitors that have added my site to their favourites?
I know with some sites the icon appears on every page that is viewed, this does not appear to happen with mine, in IE atleast.

No. Favicon is requested by Netscape and Opera to display in the address bar for any page request. Not sure about Konquerer.

MSIE only gets favicon when it adds to favorites though.

I get an idea of favorites add & return rates by filtering for favicon requests from MSIE browsers and all visits with a bookmark referral. It's not exact but useful to know as benchmarks for comparison.

I usually figure around 80% of my favicon requests are people adding my site to the favorites. It is close enough for me :)

It may also mean that iconsurf is hotlinking to your favicon - at least that was behind a surge in favicon downloads at one of my sites.

Safari (Apple's browser on OS X) also requests favicon to display in the address bar.
