I had been searching with preferences set to 100/pg for my favorite keyword. The top 26 results were all double domain (i.e. same site two results, second one indented. As a result my site was in 28 position. When I change my prefernces to 10/pg my position moves up to 20 because only four sites are doubly listed (and several sites are listed twice independently).
Here is my point and I don't know if people are aware of it. RESULTS VARY WITH YOUR RESULTS PER PAGE SETTING!
The search was on www2. I'm off to go check Google.

In short, yes, it's always done this.

Let me guess. If two pages from the same domain are on the same page of results then they are grouped together and when I switch to 100/pg their are more that show up on the same page?

Yeah either way the results are different between picking 10 and 100 results. Not sure why, but they just are.

Google will have their reasons for this...though I'm not entirely sure what they are :)

Either way I don't really think it's going to affect your side of things as a webmaster.
