What is duplicate content?

What constitues a page that has duplicate info?

What are the limits? How much information do you need to change before it is NOT considered duplicate content?

I often here you guys talking about duplicate content, but I also often wonder where the limits a drawn. I would like to here what you guys have to say.

Here is our pretend site:

Metas
- Title
- Keywords
- Desciption
H1
H2
Picture
Description (Paragraphs - 2 to 3)
H3
Another Paragraph
A few Links

What pieces of the puzzle need to be changed in order for it not to be considered duplicate?

On a non-scientific and totally paranoid basis, I make sure that all the tags are different and at least 30% new content, with paraphrasing of what remains.

But that may be overkill.

there is a lot of paranoia on duplicate content and rightly so.
I wish Goolge would be more clear about these things.

A WMW site search on "duplicate content" will make you even more scared.
