I have a client site that has no home page cached in Google's index. There is a page cached for the domain itself [mydomain.com...] Can anyone comment on what impact a missing cached index.htm or default.asp or whatever, will have on ranking?

Its ranking performance is the worst I've ever seen and it has a page rank of 5. It really shouldn't be performing that badly.

That's not your problem; that's the way it should be. All links to the home page should point to www.example.com NOT www.example.com/index.htm.
