Not in Google, but everwhere else

Bot hasn't visited in several months

For my specific keywords I typically show up in the top 3 on all search engines except Google. And the Google bots have not visited in months. I can do a search on main url and it does show, so I know I'm in the system.

My PR is a 3 and I'm not listed on many high PR sites. Could this be affecting my listings that much or could I be penalized and still be in the system?

You can be in the index and still have a penalty but it doesn't sound like it. If you're PR3 it may just mean getting some more quality links. And get a certain proportion of them to link to you with the keywords in the anchor text.

Also - get some that are on topic, with fairly good rankings if possible. And some listings in good directories wouldn't hurt.
