Site Gone From Google? Any suggestions greatly appreciated.

My index page had been indexed by google. I could find the site listed in the top 5 by a key phrase. I could also find the site by searching by domain name. Now I can locate the site searching by key word or domain name?

Any ideas on why this could be? Has anyone experienced something similar? When will she come back?
