About two months ago I updated a page within a site for content that was included on that page. It had a PR4 but since the update the PR has been removed. Is this normal and should I expect the old PR to return?
I am afraid I may be penalized because not all of the meta Keywords are present on the page.

The page title and the meta keywords tag are two different things. There's no reason to have words in the keywords tag that aren't on the page but that's not likely the reason.
What's the linking situation to your site - and more important, has there been a major change in rankings?

The rest of the site has retained its PR of either 4 or 5. Just this one page is not showing a PR and I believe it was since I updated the page title. Is it possible the page is penalized for some reason or is it just going to be re-ranked later?
