Should a link to the site map be included only on the index page, or will it harm anything if it's on every page?

Hello and welcome to WebmasterWorld

I would be inclined to link to it from a common navigation or at the bottom of each page. Think about the user as well as the search engine spider. Sitemaps as far as I am concerned are useful to the user (I use them) to help them find their way around. By linking to it you are making it easier for people to use your site. If it were dodgy to do it then problems would arise as a result of having a common navigation system on all/many pages.

Agree with ukgimp, stick a link on the bottom of each page.
