Proper 404 Error Pages

OK in the SES thread it mentions the Error paes (ie 404, 500 and so on) need to be proper error pages with header info and not just a normal substitution of html. My question is, how is this done in normal .html or .shtml? Can someone post an example of a proper error page with headers anda ll?

My hosting compagny ask me for a $75 fee to setup a custom 404 page.
I think it's not a real problem for most people but for me it's $75 to increase my Pagerank!

What does a 404 have to do with PR?
