Special e-mail forms, Real Special!

A little info on e-mail forms needed......

Let me explain, I have an e-mail form for vistors to submit information, as an example. Before they click on submit, can I have a disclaimer pop up asking if the agree to specific terms? They will have to initial the disclaimer or type their name at the bottom in a text box. I then need the information along with the electronically signed disclaimer sent to me. Has anyone done this, can you shed some basic knowledge on me? Thank you very much!

Why not just add the signature field to the main form instead of having a pop-up? I for 1 would be really upset if I took the time to fill in a form and was asked to agree to terms that I may not agree to.

Thats true, it may make it easier to receive the agreement initals that way as well. You would just make sure to verify they filled in that section if they dont it kicks them back saying they must read and fill in the info.
