I have a content site and have dabbled with banner ads in the past, but I'm not really sure I've been getting the best deal. BTW... our traffic is strong at about 20,000 page hits a day.

What CPM should I expect to get from an ad service?

How is CPM counted (per 1,000 unique visitors or page views... I can never get that straight)?

Which services are best?

CPM is based on pageviews, not on unique vistors.

We have our own portal with different pages and also use different advertising 'agencies' to maximize our income stream. By far the best program we have experienced for CPM advertising is FastClick. It is really easy to use and you can decide for yourself which ads run where. You will be paid monthly based on your traffic.

You also might want to look into WebSponsors and Commission Junction but those programs are more CPC and CPA based ad campaigns.

Similarly, we use Fastclick for banner adverts, and also Casale.

CPM varies widely with advertiser and campaign, but as an average we realise eCPM (after the agency takes their percentage) of $0.43. Individual campaigns vary between eCPM of $1.60 down to $0.03 - needless to say, we don't carry very low paying campaigns.

Casale, by contrast currently provide us with an eCPM of $0.64 for banner adverts.
