I received my amazon 1099 and it only includes Q4 2003 and nothing for Q1, Q2, Q3 2004 which I earned income. Is that normal for them to do that and will I receive 3 other ones from them or?

looks like they changed their name and in-turn are sending 2 1099's out.

Yes, I got two forms on the same day. One covered the first payment I received during the year, and the other covered the other three payments. So you'll probably get another form shortly.
