Help Needed For Meeting With Advertiser

I'm currently talking with a representative of an ad network and she sent me a list of questions about my site that I need to awnser before I can join as a publisher on the ad network. I'm not quite sure what to awnser for some of these questions:

What are your basic demographics?
Do you have an adserver?
Do you have a sign up process for membership? (I'm pretty sure this means registration form, just in case I'm putting it here)
What media types are you willing to display on your website? (I think this is asking me if I'm willing to put up casino ads and stuff like that)

I also want to know which program I should use result from for daily uniques and daily pageviews questions, I currently use webalizer, will that give accurate enough stats for them? It also asked me for what cpm and cpc rates I am looking for, what is the average cpc and cpm rate (if possible give me a little bit below what you believe is average, I don't need alot of money, and I think it will help if I want a small amount).
