Create Forms In Flash using PHP

Hi, this is my first post, so please be easy on me!

I have cracked basic forms using Flash, i.e. Name, Address, Contact Number Fields. But I now want to add combo boxes an radio buttons.

I have added them to my designed page, but i can't get the information sent from the site to my email.

Anyone have any good ideas? or tutorials? or just any help at all?
