How can I discover what is the position on the Google SERP of my web site/page for a specific keyword/keyword phrase? Is there any tool?

There are lots of paid tools out there, some offering greater scale than others. A quick Google and you'll find them.
If you're only going to check every so often then doing it by yourself makes sense, providing you understand how to fetch the results you want, i.e. query, language, location, google TLD.

If it's a one time thing, then I recommend an extension for Google Chrome called "SEO SERP". You just type the keyword, along with your domain and it will check the SERP to see if you're ranking for that specific keyword.
Another tool would be for Firefox called Rank Checker. This tool allows you to check an unlimited amount of keywords. (as long as Google doesn't block you for too many queries)
