On-line questionnaire

Not sure if it is exactly the right forum to ask, but it was the nearest I could find! What I want to do is have an on-line questionnaire to find out more about my users. Age, average income, times out a week they eat out, that kind of thing. It is also pretty much 99% UK based. Has anyone come accross any good references, geared to the UK, of standard information to gather?

What will be the purpose of the information you collect? I would start there and work backwards to determine the right questions.

To use to show to potential advertisers about the target audience they would reach.

I'm afraid I can only give you a quick overview of what U.S. marketers like to see when determining whether or not an advertising medium is the correct audience:

Age range
Household income
Spending habits revolving around their products (how often do you...)

...and the important one...
Would they be interested in learning more about (insert your affiliate program here)
