Try putting the tracking code below the form. Often these companies ask for the code to be at the top of the page, but this can cause problems, especially with load response times. If your form uses some dynamic technology, this might be causing the problem. (I am not a coder - so don't shoot the guy just trying to help...)
The more complicated solution would be to stript the tracking URL extension into a cookie, then feed it back into the tracking script by altering the script to take its data from the cookie, not the URL. Again - I just ask people to do these things... I can't do it myself without hurting my head.
