Does IP Targeting actually reduce stray clicks from non targeted regions? Is the difference in clicks significant?

Sure! IP targeting these days should be well over 99% accurate at the country level. At more local levels, targeting becomes less accurate, though. I am currently "out" by about 50 miles on my IP number.
