Requests denied why?

Curious why some requests are served and others refused. Everything looks the same. Is it that the requests are so fast that some are denied?

Error log says: client denied by server configuration

Could be. 403 = Permission denied, which can happen if too many users are browsing the site. Depends on your server's configuration.
