Is there any off-the-shelf program we can use to create an affiliate program that anyone has used and is working well.

You are looking for software correct? If so, it would be best to take up the 30 day free trials that many offer to see what works best for you and if it has the features you are looking for.
I don't know what would turn up, but I would look for " affiliate software reviews" and affiliate software comparison" to get an overview of what's available.
