Extended: Transactions may be extended by an advertiser one time only, for a period of 30 days.
Does that mean they hold of paying the publisher for 30 days?

An extended transaction is not going to hold up your publisher payment - that is unless 100% of your transactions are extended.

What does it mean? Does it affect the publisher in any way or give the merchant a 30-day window to change the transaction?

This happened to me about a year ago with one of the better know toy stores on CJ. It turned out that the toy store wasn't paying CJ because they were going through chapter 11...that is why they put my sales in 'extended' status. In the end, I didn't get any money from them.
