CJ for U.K. sites

Do need to complete W8-BEN form

I am UK based and I have applied to CJ via ww.cj.com, but am a little confused by the W8-BEN form which it seems I have to fill in.
As a U.K. citizen,do I need to complete it, and if so, which boxes should I tick?

Yes you do need to fill in the form but I don't remember which bits. It was a long time ago.
