Can someone send me an example?

I am tryng to design my invoice based on a MS Office template, but am unsure of what exactly to include. Could someone possible give me a visual example? I'm a very small business and do not need to charge sales tax yet, so it can be pretty simple.

Any help would be super appreciated!

I'm not quite sure either ,but the basics such as price, dates, customer, tax id, number of impressions etc. I'm sure you could probably find some examples laying about using a serch engine also.
