How is this form done?

I have been asked to produce a similar "contact form" to the one in this example. The example is essentially an quote form with several different options for the product in terms to shape, size and material.

(each category has a slightly different version of the form based on the product selected)

Was this created by hand, or does anyone know of a plugin that would handle this automatically? The site is based on Joomla - 1.5 I think.

You can create dynamic forms using either a server side script such as php, or do the work client side using js.

As a user makes a selection only show the form that is relevant, there may well be plug in scripts for your cms.

thanks for the reply, was just interested if there was an "advanced form" type plug-in that already existed for Joomla that would handle the example.

Looks like they had it done by hand then.
