Create an XML sitemap that updates automatically as you add pages and submit it through webmastertools and forget about it.

I think it depends on how often you add new content to your site. On one of my sites I update the site map hourly to let the SEs know about new forum posts and such. On another site I only need to update once a week. On still other sites I never need to update. As Made suggested, automate the process, especially on your busier sites. I wonder if there's any benefit in updating a site map every so often even if there's no, or no substantially new content.

I update my sitemaps whenever i add or change pages on my site. Makes it easier to make sure that the pages are there, rather than updating it once a week and forgetting about a page i added 5 or 6 days ago.

But i mostly deal with smallish sites. Although i would like to know how to make an automated xml sitemap..i presume we're talking about xml sitemaps?

You should update your sitemap when you want the new info. to be available through your sitemap.
