The basic form doesn't work as expected.

I went through the form you had provided and it looks like you have used some software or a template for your site development.

This is the portion of the code which will send an email to the designated person.

Can you tell me what is the exact error you are getting?
Also, let me know what do you intend to do with the form. I mean what exact functionality you want with the form, like email to a person and then jomp to another page or anything else.
