Why is my website experiencing low local rankings?

Hello everyone! I am an SEO freelancer and am currently working on an infertility-related website. I observed one thing: my website is experiencing low local rankings. I have been doing local SEO on my website for the past 3 months and still failing to rank my website even on the 2nd page. Please give me some advice on how I can improve the website ranking.

Is the content related in any way to local interest? It can help when local places and events can be used within your content to help identify an emphasis on the location within different parts of the site.

Yes, the project is IVF related and I am targeting local locations and community for the past 3 months and still failing to rank the website.
