i know these could be some strict requirements, but being an InterDev user my whole programming career, i have no idea what's out there now.

It has all you can ask for, although I've not used the ftp capacity.

Yes, as Condor12 said, HTML-Kit fits the bill nicely. I've used the FTP capabilities quite a bit. Documentation is a bit scarce, but it has loads of features.
And you can't beat the price!
It wasn't until a fairly recent version for example that the programmer figured out how to do text indenting in blocks.
It is free, that's the best thing it has going for it.
Edit Plus works fine, you can configure the code templates as much or as little as you want, or just make your own, that's the easiest.
I'm still looking for the perfect code editor, I keep trying various ones but I keep going back to edit plus, if you take the time to learn how to configure it, it's pretty effective.
Excellent built in FTP, and although it doesn't automatically do the DOCTYPE insertion etc. you could easily set-up a macro so that it does.
30-day free-trial, but definately worth a look at.

Each to their own, I guess. I kept trying to find a better combination, but can't find a better one than dreamweaver for site maintainance, link checking, sitewide search and replaces, and so on, and editplus for everything else.
I tried, several times, several builds, there were always core programming problems that indicated very clearly to me that the main architecture of the program was flawed, I emailed chami about this and he pretty much admitted that, he's in the classic problem of weak initial architecture of the program leading to extreme bloat, but never being able to be fixed without a complete up from the ground rewrite.
He keeps adding all these interesting but fairly trivial features, many of which often didn't work, or were incorrectly executed and led to my system getting messed up, making the code more and more bloated, but the fundamentals don't change.
What did it for me though was that ridiculous search and replace plugin that crashed the first time I used it, there's no way I'm going to let something that poorly done do any large scale searches and replaces. And it didn't even do multiple line search and replaces anyway.
Plus it's super bloated, editplus is just a better written program, too bad it's not being actively developed anymore, but it doesn't really need to be, it's clean and simple, more features than a simple text editor, not so much junk like most other html editors out there, I use it as my default html viewer in opera, and it opens almost instantly. I tried doing everything I could do with edit plus in htmlkit and nothing would ever work right, mainly due to the weak original design. He might fix that one day, we'll see, this was just a school programming project for him, very good for that, I'd have given him a B+ if I was his teacher.
I agree about Dreamweavers' site-management - I certainly miss that.
As an ultra-fast source-viewer and editor, I've recently started to use Scintilla - So far quite impressed.
Also, I quite like a relative newcomer on the block, LessThanSlash which is small and fast with FTP and a handy basic image-editor...also good for lecturers teaching XHTML.

A possibly interesting product - but, unfortunately, it has no Help system, so you can't actualy find out what it does and how to make it do it (whatever "it" may be). This is a serious failing, which hopefuly will be rectified fairly soon.
If you go to the "proper" site, the one that has .info as it's tld, there is better help there including a flash-based tutorial.
If you have any editor-based experience though it's not that difficult to pick up.
The editor is the important component IMHO, 90+% of your time is spent thinking in front of it! Getting the bad boys to the server isn't really a big deal, and all FTP programs I have ever seen screw up in some way shape or form!
mattglet - have you ever considered dreamweaver?....ROFLMAO....nice to know there is someone else who feels that way about it!
