Regarding Frontpage Form

In need of help to make the form works!

Hi, I have built a form using Frontpage and when I go to the form properties dialog box, I am at a loss in providing an accurate path for storing the form results (Kindly refer to the following):

Where to store results:

How do I type in the exact filename or path to make sure the form is sent?
2) How do I test the form? [example.com...]

I am really clueless and in need of assistance. I hope someone can offer some insights. Please help. Thank you very much and I appreciate your time taken.
