I've always used broad matches, is it worthwhile using the others as well ie. for every phrase have 1. blue widget 2. "blue widget" 3. [blue widget]
it's massive duplication but is that a better way to have all three listed so they perform better and cover all search types?

I've found great value in splitting out match types.
Once you test this on a smaller segment of your campaign, you'll get a better idea of which keywords make sense to split out and which ones don't - helping to somewhat limit the massiveness.

Well, there are limits to the # of campaigns allowed in an account, # of ad groups allowed in a campaign, and # of keywords allowed in an ad group.
The limits depend on what "type" of account you have, so you'll need to develop a structure to accomodate for all the additions.
