How does one get the big blue box at the top?

It seems that conversion from the blue box would be much better than from the side.

It's just my 3rd week with adwords so thanks for all the help so far!

You can get into the position above the free results with a high enough bid, a strong clickthrough rate, or some combination. The tipping point would be different in different sectors, because it would also depend on what the competition was or wasn't doing right.

I'm a cheapskate bidder so I'll never get there by brute force bidding, but I've had a few ads with a CTR of 15% or better go above the free listings.
