I see the very top spot as a status thing for my product. I seem to make better money in the list to the right. Probably 'cuz my product is small, used infrequently, and very inexpensive. It would cost me far to much to try and take the top spot and still make money.
I think of it this way, like comparing two fast cars you are interested in buying.
Why pay this much ---->XXXXXXXXXXXXXXXXXXXXXXXXX<---- money
for only this much ---->XXX<---- increase in performance
Your over all cost might end up being quite a bit higher than the the roi difference you see from switching 2nd place to 1st place. I'm gussing there are just as many eyeballs seeing your ad in 2nd place as there are seeing that top spot... maybe... I could be wrong... i usually am. :-P
On the other hand, if you can stand to keep paying for that top spot for a little while, and it does begin to pay off by showing a better ctr, then you should expect the cpc to come down to hold on to that spot.

I agree - positions 2-5 convert way better than 1. In position 1, you get a lot of people clicking on the ad just because it's the first thing to click on. In the right-hand positions, there is a greater chance of users reading your advert and clicking because something attracts them.
