Not being defensive for them, but what link/relationship do you have with Google, that you registered a domain with their brand in it and then proceed to advertise on their site.

First, regarding "online" on the ad title. Usually this is a straight disapproval, BUT if you use online in the title promoting a specific service that is online then it should be OK (such as "online gaming" but not if you are simply selling a product online).
And as for ads being disapproved because they have the same word on two different lines - this is not only due to the repetition issue but also because of consecutive repetition of a word/phrase that ends on one line and appears at the beginning of the next line.
Example:
Buy new widgets.
New Widgets from Timbuktu.
Get them Here.
www.afldkfjslf.com
On AOL and some other partner sites, the ads appear on one line, so this one would look like:
Buy new widgets. New Widgets from Timbuktu. Get them Here.
So the ad would be disapproved for having "new widgets" consecutively and looking bad on one line.
One of the main reasons behind many of Google's editorial policies is simply how the ad looks aesthetically. It's a very important part of PR that can't be overlooked for Google. Their own image is directly reflected on the ads they show on their site.
Google prides itself on the quality of their search results, and so they naturally have certain expectations along those lines for the ads that appear on their site.
