Bid high and write great copy. The ads that rise to the top are those that make Google the most money. The ad with the highest ad rank (bid price * click-through rate) rises to the top. In most cases, the ad position is always in flux so there really is no way to occupy one of the top spots 100% of the time.

It takes time. After the required number of clicks or impressions, if your CTR is great, you can get up there.

It's extremely hard to get to the very top from #3 or worse in the right column - so starting at #1 or #2 in the right column is very important.

Perhaps you'd prefer a simpler reply: How do you ensure your site comes 1st or 2nd in adwords? YOU DON'T.

Welcome to the competitive arena that is Adwords :)

Keep changing your bid until it appears at the position you desire.

(then monitor daily).

It certainly is an art, a surreal one. I have one Ad Group where I made it to the top with a relatively modest bid, but CTR of 5%. My CTR went to 10%+ when it hit the top! What's perplexing is that another several similar Ad Groups are coming in around 1% with ad text that's identical except for the substitution of the corresponding keywords. Beats me, and annoys me too because the other keywords get 10x the impressions.
