Lets say you have an Adgroup with 1,000 keywords and 200 of those have been Disabled.

How do you get that batch of 200 reinstated? Is there a cost? I have read about a $5 reactivation cost but is that per Adgroup or per keyword?

The 5$ cost applies only when your account has been slowed.

If your words have been disabled, you could try (but there's no guarantee) deleting them everywhere in your account, and then re-insert them in a different adgroup.

If your keywords have been disabled, you cannot reinsert them again for that account

Edit keywords, copy all, paste them into a text document, cancel edit keywords, delete disabled keywords, edit keywords and copy them back in from your text document.

This ONLY makes sense if you have improved other things in the adgroup like better ad copy, higher max CPC, add negatives (depending on what may be needed for those keywords). Otherwise they won't be undisabled for long.

It's better to put them into a different adgroup more ajusted to those keywords.

We spend as much as 30K a month in Google AdWords for a cosmetic product... After many unsuccessful emails and phone calls with Google over several months, we gave up and opened another account just for two words. That new account is doing wonderful and Google is aware of the account. I don't think anyone in Google can actually flip a switch and turn a word or several words back on. I think there are many of the Google reps that wish they could because it seems so unfair to permanently disable terms in legitimate accounts who are trying to do "everything right" and just haven't got it figured out yet. There was a time a few months back when disabled meant "0" impressions but lately I'm noticing that my disabled terms are getting a impressions few here and there.
