Conversion Tracking Not Accurate

We recently changed our information website, and the 'Contact Us' form (where we track conversions) went from a standard html page to a java pop up.

We're using the same exact pixel code but now, all of a sudden, adwords is not capturing 50%'ish of our form completions. The conversions are consistently down (for example: we had 40 info form completions in the first week of June, but adwords represented 21).

Has anyone experienced any issues with the G pixel and java?

Haven't seen conversion issues tied to java, but I have seen a huge discrepancy when you use a non secure conversion code on a secure page, or vice-versa.

This is probably a more obvious one, but I've seen it happen many times: Google is extremely picky about their conversion code, and I've seen cases where a line wrap where it shouldn't be can sometimes mess everything up. Could be worth re-generating the original conversion code, and making sure it's accurate on your target page - line-for-line.

Hopefully one of those quick-fixes helps.
