I have a couple of KWS that are shown on my ads headline and descripion. Is a perfect match in ecery way! Whay the hell can't i get my QS over 4?
I tried tweeking the ad copy, still leaving the KW on the headline and on the body, but nothing... And now they boosted up the cost for the KW by 40%.

It is important how you put your ads, but that's not all.
Google is way above the linear thinking in both paid and organic search. Rules like where you put your keywords, etc, alone - will not give you the best result.
I believe that G marks your site right up front (or within few days) and there you go.
Just like regular search algo takes over 200 factors into its calculation, AdWords does something similar for sure.
At the end, the site should be useful, however one would grasp it.

QS is highly related to CTR. It is also calculated by position and a measure of how well you are doing against competitors, historically I might add, not just currently active advertisers. Therefore, your QS=4, just above the old Poor rating, assuming your keyword is deemed relevant, means your CTR is not that good compared to competitors. Try to improve your ads for better CTR.
