Ad shown as being triggered by wrong exact match keyword

After finding a few problems regarding keywords in a campaign I decided to go through all the Adgroups/Keywords so that I may find and fix any problems. However, I have come across a problem that I cannot resolve as I cannot find the source. Using the ad preview tool I typed in a keyword EXACTLY as it is in the adgroup (I even tried copy/paste).. but the ad was shown as if it was triggered by a different keyword.

Example:
the adgroup contains both of these keywords (well, these are just examples not the actual keywords)

[Birthday Cakes w/ green frosting]
[Birthday Cakes w/ blue frosting]

The ads for the adgroup uses keyword insertion in the headline, with the default being just 'Birthday Cakes'

For some reason, when I type in Birthday Cakes w/ green frosting (even when I surround it with " " or [ ]) I am shown the ad with the headline of "Birthday Cakes w/ blue frosting"

Any Idea of what could be happening?

As I understand it, Google doesn't always serve
the ad from the ad group that your keyword is actually sitting in. Their system ultimately determines which ad is relevant for the search query. If it finds another ad it deems "better", and it happens to be located in a different campaign and/or ad group in your account, then it could serve that ad.

I dont think you seem to understand what I have said. This has nothing to do with it showing an ad from another adgroup. the problem is that the ad is showing as if it was triggered by the wrong keyword in the same adgroup. They are both EXACT match. There is not supposed to be any room for interpretation as far as what is "better" If I have an exact match keyword for 'Birthday Cakes w/ green frosting"' it should not be giving results for 'Birthday Cakes w/ blue frosting'

The easiest solution would be to break this into 2 ad groups and add the respected colors as negative broad matches. This would ensure the correct ad is displayed. Dynamic ad titles are great in some situations, but can be troublesome in others.

Have you tried ad diagnostic tool? Just so you see what's happening at the back end.

It does not sound right to insert keyword that has not been typed in. In any case, if not the keyword itself, you would expect the default to show.

ppcbuyers is right especially if you have a granular adgroup structure. It sounds like you do. i deal with this on a daily basis. ppcbuyers is actually right. At least that is what I have been told by Google as well.

Yep, that's been my experience as well.
