What on earth can be the reason for targeting a site with an ad like that?
Do some advertisers not know that publishers can approve or reject site targeted ads?
Sadly, this was an advertizer that fit my site really well, and now I blocked all ads from them.
I sure don't want ads like the above running on my site if I can help it.

It may have been more of a 'misfire' rather than an attempt at targeting.
Perhaps someone performing a test to see whether their QS was being influenced more by the the landing page as opposed to the ad? Maybe testing to see whether a tracking string came through.
I've done 'quickie' tests not so unlike your example but hopefully never left them active.
