If I make a change to my destination url, does it have to be reviewed by an editor before going live?

Yes, this will require a review before running on Search Network or Content Network. As with any other ad change, it will continue to run on Google Search pending review.

Note that you will also lose your ad's "karma", as ANY edit to an ad actually deletes the old ad and creates a new one. It's considered a NEW ad, with NO history.

You can avoid the "karma" issue by changing the URL for the keyword, rather than for the ad. Will still require keyword review before running on Search Network of Content Network.

Each keyword can have it's own destination URL, independant of that of the ad. If a keyword has a URL, it overrides that in the ad.
