Why isn't my ad showing up?

I've got a 9 year old site that I finally decided to monetize. I placed one ad there 3 days ago. It's working fine.

I went to add another ad today, and it doesn't appear at all. No PSA, no nothing. I checked the code, and it seems to be all there. Both ads are in SSI-included files. I know the include works, because other content in the included file shows up just fine.

When I open the "include" file in my web browser, the ad DOES work, but not when it's on the live site. Viewing source code shows it there OK.

Any ideas what might be going on?
