AdSense click / IP tracking script

I know that the adsense click / IP tracking script has been discussed a few times here, I have read most of the documents but they contain many code snippets and not all of us can easily use it.
Can anyone copy and paste here the code snippet that can be used to track the IPs clicking on adsense ads? Some, brief explanation of how to use it is also welcome.

I use Asrep (search for it). This is low cost and very efficient tracking tool. Well worth the money IMHO.
