I've been trying to track the evil words causeing the PSAs, by line for line censoring likely miscreants and waiting 2-3 hours with a new url to get respidered and check.
Now, this line has me stumped:
"You will learn how" works, but
"You will learn how well" shows PSAs, all else beeign the same (except a different number as url)
So I did a little experiment, I created a plain html file with a little targetable text that is certain to have ads available:

<p>This is an innocent looking page about <strong>hotel booking and travel</strong>. It will test what words case the Google AdSense system to go bad and show PSAs. The results of this test will be used on <a href="http://domain.com">My Website</a>, to provide better information about treavellign and hotel booking.</p>
Then, I placed the bad word after the </h1> tag. Low and behold, ALL the evil words resulted in trargeted ads, EVEN those that the AdSesne support team told me EXPLICITLY that wouldn't work, including "death" and "heart attack", in fac the very first ad for "deaths" read "Death Records Search", while "death" brought ads targeded to the keyword "adsense". But NO PSAs across the board, INCLUDING the words I thought I had already confirmed, such as the infammous s*x as well as it's cousin "s*xual health" and "pe*n*s".
Could it be word density? But in these short pages, the single BAD word should be overpowering, as well as previously adding a single inconspicious looking word to the health page immediatley brought on PSAs.
I will further investigate, but I must admit I'm somewhat baffled.

SN, in your tests, I'm not sure they're really accurately measuring AdSense targeting, because from my experience, I get different results at these periods:
- Initial upload of page
Targeting based purely upon general site topic and/or URL (e.g., a URL like mysite.com/gifts/bluewidgets.htm will show ads about blue widgets... OR ads that match the overall known theme of mysite.com or 'gifts')
- Later that day and/or the next day
Mediabot has stopped by (sometimes within minutes!), and targeting is based more largely upon the context of the page.
- Days or weeks later
I find that the targeting changes once again... sometimes to more refined terms, or on the opposite end, to PSAs <sigh>
So I don't think you can make assumptions based upon immediate behavior with AdSense targeting.
